2026-2027 Sports Physicals

We will not be using Aktivate for next school year. Please hold on to your completed physical evaluation until we have more information from the district. You can also email a copy to a Campus Athletic Trainer if you think the original copy may run away this summer. Please make sure the 2026 version of the UIL physical form is used for your appointment and that the evaluation date is on or after May 1st, 2026. Please complete the entirety of the first page, including the student and parent signature sections prior to your visit, and make sure the doctor completes the second page in its entirety prior to leaving their office. The link to both the English and Spanish versions can be found below. We'll also be providing more information about free physical opportunities soon, so please be on the look out for that!
